Ayushman Bharat PM-JAY
Central scheme
Cashless and paperless health coverage of ₹5 lakh per family per year covering over 1,500 medical procedures including surgery, medical and daycare treatments. Pre- and post-hospitalisation expenses (up to 3 days before and 15 days after) are also covered. The scheme follows a 'family floater' model — the entire ₹5 lakh can be used by any or all members.
Eligibility Criteria
- Families listed in SECC 2011 database under D1–D5 deprivation categories
- Automatic inclusion for Rashtriya Swasthya Bima Yojana (RSBY) enrollees
- No restriction on family size, age, or gender
- Available at empanelled government and private hospitals across India

- PM-JAY e-card (generated on portal after eligibility check)
- Any government-issued photo ID
- SECC 2011 reference number (if available)
